Guidance notes for completing our online job application form

We have listed below all the questions on our application form, so you are able to prepare the information before inputting it into our online form. A copy of your responses will be emailed to you once you submit the form.

Please note that we do not accept CVs, only applications made on our application form.

Section - 1 Personal Details

Sections 2 to 6 – Equality and Diversity Monitoring Form

We are committed to being an equal opportunities employer and recognise the value that a diverse workforce can bring.

The information requested on this form will be used to monitor equality across all aspects of Sistema Scotland’s processes and used to:

  • inform future practice
  • understand whether we are encouraging equality and diversity
  • provide anonymised statistical information in funding applications
  • meeting the aims and commitments set out in our Equality and Diversity policy 

We need your help and co-operation to enable us to do this, but filling in this form is voluntary. The form, and the wording in it, is based on the current recommended best practice template on the ACAS (The Advisory, Conciliation and Arbitration Service) website.

The information you provide will only be used for anonymised statistical reporting purposes and will be stored securely with access limited to only staff in the HR team. We will ensure that all monitoring data that is collated will be managed confidentially at all times by those involved and will be done in line with the General Data Protection Regulation (2018).

If you are a job applicant, this information is not seen or accessible by the selection panel and does not form part of the selection process in any way. It will be retained in anonymised form for three years.

If you start work with us (or are an existing member of staff), this information will be transferred to your HR record and will be held in the strictest confidence for reporting purposes only.

 

Sections 7 to 11 - Employment History

For each employer, starting with your current (or most recent):

  • Name and address of Employer
  • Date employment commenced
  • Date employment finished (if applicable)
  • Position held • Summary of main duties and achievements
  • Current salary (or salary at date of leaving) – this question is only for your current or most recent employer
  • Reason for leaving (or wishing to leave)
  • Notice period (if applicable) – this question is only for your current employer

 

If you have any other employment history details to add after you have completed all the pages in Employment History, please include them in the section "Personal Statement in Support of Your Application".

 

Sections 12 to 15 - Education and Qualification Details

From secondary school onwards, for each qualification, or set of qualifications:

  • Name of school, college, university etc
  • Date commenced
  • Date complete (if applicable)
  • Qualification(s) gained and grades (if applicable)


If you have any other education/qualifications details to add after you have completed all the pages in Education/Qualifications Details, please include them in the section "Personal Statement in Support of Your Application".

 

Section 16 - Membership of Professional or Regulatory Bodies

  • Please detail any memberships of professional or regulatory bodies.



Section 17 - Professional Performance Experience (for Musician posts only)

  • Please detail here any professional performance experience (for Musicians posts only) relevant to the role applied for, including dates


Section 18 - Relevant Voluntary Experience

  • Please detail here any voluntary experience relevant to the role applied for, including your role, the organisation and dates. 



Section 19 - Personal Statement in Support of Your Application

  • This is the most important part of the application form, as it is your opportunity to explain to us why you feel you are suitable for the role you have applied for. Please refer to the job and person specification for the role, and demonstrate how you meet our requirements. 

 

Section 20 – References

Please provide the contact details of two referees. At least one, and ideally both will be employment referees and must include your current or most recent employer:

  • Name (and post title if applicable)
  • Postal address
  • Telephone number
  • Email address
  • Capacity in which they know you
  • When may we approach your referee for a reference? 

 

Section 21 - Declaration

You are asked on this page to agree or disagree that you certify that the information contained within this form is true and accurate to the best of your knowledge and belief, and that you understand that any information that is knowingly withheld, deliberately misleading or false, may result in dismissal if you are employed. 

You are also asked to agree or disagree to consenting to your details being kept confidentially and used for specific and lawful purposes as specified in the Data Protection Act 1998.
